Problem with CUSTOM FIELDS

    Dear colleagues,
    I have a problem with the newsletter subscription form. I thought it was due to an undefined problem with the module, so I installed a BLOOM as well. The same thing there. I'm sending a screen, notice that they are only a choice:
    - Address
    - Birthday
    - Phone Number

    In the here-right.png file you can see what options should be and what options are not there for me. Please help me :(

    On the right.png, you're using a contact form module, not email opt in module.

    Contact forum module does have lots of custom fields, but it doesn't allow email subscription. That's different from email opt in module / bloom.
