I found that I have to disable DU header and enable to see the changes.
It's is not refreshing after adding almost anything with margins, padding, colors, icons and so on. menu items added in the menu not affected.

It should work just fine. It's just that our custom menu module is partially supported in Visual Builder, but it will still load fine in Visual Builder.

Normally, you will only use this custom menu module when editing custom header in Divi > DU Header - Library
**And we did hide the message when you're editing it in Divi Library. Though, if you updated it to Divi v4.0, it changed some of the HTML structure, which causes the error to show up again. To hide the error message in Divi Library in Divi 4.0, you will need to re-download & re-install header plugin v3.0.1 (which we just updated to fix it few days ago): https://divi.help/threads/divi-ultimate-header-plugin-v3-0-1.3426/#post-11177

    ===========

    Extra Information:
Alternatively, you can also use the default Divi regular menu module in Divi 4.0. Just copy the CSS Class in our menu module settings & paste it to the default Divi regular menu module & adjust the design options to fit your need. We already updated it to work with the default regular menu module in Divi 4.0.

I have tried the next zip file sent and no change.

    e.g.

I change the text color from red to white and I save it. I hit refresh and no change.
If I disable the plugin and enable it...then the change takes affect.

It's like it not recognizing the refresh?

This has been an issue since divi update 4.0.11

I do not think its a plugin issue, because if I go back to divi 4.0.10 or earlier the problem goes away.
    with the same plugins running.

Sounds more like a cache issue.

After saving it, try to clear your cache & see if it works:
    1. Cache Plugin
    2. Static CSS File Cache: WP Admin > Divi > Theme Options > Builder > Advanced > Static CSS File Generation > Clear
    3. Browser cache

Also what about updating to the latest Divi version 4.2.2?
