Solved Help with contact form - get the Url page

    Hello world !

    I am using the same form on multiple pages of my website, when a user fill in one of the form : I would like to know the page from which he sent it from.

    Example
    "Page A" and "Page B" both include "form 1"
    User fills in the "form 1" in "Page A"

    I want the email that I receive to say "form sent from Page A"

    It must be really simple only this information is nowhere to be found :/ help ? :D

    You can just add a new Radio Buttons field & set it like the below:

    And lastly, set it be have such conditional logic, so that it won't show up on your page:

    That way, when the user submit the form, it will include the radio button message as well.

    Thank you for your quick response !!

    I may be missing something but the email doesn't include the radio button message.
    I tried leaving the "email template" empty & tried again with adding %%ID of the field that include the radio button%% but none of them gave me result :/

    Edit : I finally found a solution
    I just put the name of the page in the email template. I cannot believe I did not think about it, it is much easier .

    Example :

    the name of the contact is %%name%%
    submitted from the page A

    that's it, bob's your uncle :D
