Form Conditional Logic Not working on Mobile Devices

I'm trying to understand why Divi's built in Form conditional logic works on desktop, but not mobile. The site I'm referring to is: https://journal.rishi-tea.com/wholesale/

Replicate the issue:
If you go to that site and select a business type on desktop, you'll see that the conditional logic properly shows the new fields. However, if you go on a mobile device, or into the browsers settings (I'm using chrome on macOS), toggle the device toolbar, and select an iphone or ipad, then reload the page (hard reload, clear cache), the conditional logic no longer works.

My findings:
So far, I see that all CSS files loaded are the same on both mobile/desktop. And the only difference between mobile/desktop javascript code is that on desktop, admin-bar.min.js and wp-embed.min.js load in. So it doesn't seem like theres an issue with a file not getting loaded.

Any thoughts?
