Gravity Forms now showing on certain pages

    Hello! I'm trying to figure out why certain forms on our website were not showing up.

    The following pages are not showing the Gravity Form:

    http://overconfident-pocket.flywheelsites.com/financial-request/
    http://overconfident-pocket.flywheelsites.com/care-2/
    http://overconfident-pocket.flywheelsites.com/food-request/

    Gravity Forms do show up on other pages such as http://overconfident-pocket.flywheelsites.com/affiliation/audition/ so I'm not sure where the disconnect is.

    Some forms use conditional formatting, but I think something is interfering with it and giving it a “display:none”.

    Can anyone help us figure out what the issue is?

    Based on your site, it's not due to the display: none; issue. It seems like the form only has this one text field. See screenshot below:
    **You might want to refer Gravity form support about it.

    As you can see from the working form, there are plenty of fields:
    **Even it's hidden with conditional logic, it will show up in the HTML. But the non-working ones only have 1 text field.
